After Care Gym Program
After Care Program
$40per month
- Includes access to our Gym during normal working hours.
- Gym Hours: Monday through Friday, 8:30am – 6:00pm
We are excited about your desire to participate in our After Care Program. You have the unique opportunity to utilize our state-of-the-art equipment in an environment where therapists are available for input and over-sight.
The goal of this program is to encourage our patients to maintain an active lifestyle through the use of our facility to obtain and maintain optimal recovery and prevent recurrence of injury.
This program allows you the use of all exercise equipment, weights, and mats; however, patients receiving treatment by therapist take precedence upon use of equipment if needed.

Accepted Insurance
The insurances we accept may change annually. If you’re unsure whether we accept your plan, we can verify your benefits for Physical Therapy by contacting your insurance provider. We also offer reasonable cash pay options.
Typically, we are not included in marketplace plans or certain HMO plans. If your HMO plan is accepted, a referral is required before scheduling an appointment. Please ensure you have a referral before calling to schedule.
An order from a referring physician before your first appointment will streamline the process for scheduling further treatments and appointments
